Northridge Care Center coordinates access to a range of medical and ancillary services, which may include audiology, podiatry, dental, optometry, pharmacy services, diagnostic testing, nutritional management, and discharge planning support.

Discharge planning services assist with coordination of care and services when transitioning to another care setting.
